I declare that the info I’ve provided is accurate & complete. And that I do not currently have Covid-19 and am not experiencing a fever or other symptons, or have been exposed to an infected person within the last 14 days.

I agree to the following: 1. I am participating in a Yoga Class that is presented by double sunrise yoga, LLC and that all yoga classes may require physical stress and involve a risk of injury. 2. It is always advisable to consult your physical prior to starting a fitness program with double sunrise yoga, LLC. I represent and warrant that I am physically fit and I have no medical condition(s) that would prevent my participation in double sunrise yoga, LLC classes. 3. I agree to take responsibility for my own safety and the safety of associated minor(s) and not engage in a fitness class I feel is unsafe for me and associated minor(s). I am fully aware of this risk and hereby release double sunrise yoga, LLC, and/or any other person(s) who may teach for double sunrise yoga, LLC, from any and all liability, negligence or other claims arising from or in any way connected with my and associated minor(s) participation in any fitness class (ie. Yoga). 4. I, my heirs or legal representative forever release, waive, discharge, covenant and agree not to sue double sunrise yoga, LLC for any injuries or death caused by its negligence or other acts during my participation in double sunrise yoga, LLC Classes. 5. double sunrise yoga, LLC is not responsible for lost or stolen property.
